CruisePlus Travel & Tours Ltd., “CruisePlus” has advised me that because of the worldwide effects of illness and changing political situations, it cannot provide any representations about any location or travel service that I am booking for my self and everyone else who is part of this booking.

In proceeding with a booking, I acknowledge that I and everyone who is part of this booking:

1. Have been made aware of all applicable booking information, including the following:
2. Acknowledge that CruisePlus Travel & Tours has no control over the performance of travel companies, the effects of illness, government policies or changes to government policies, and cannot be held liable for such.
3. Have had medical advice and are fit to travel.
4. Are aware that passports are the required form of identification, that our passports must be valid for 6 months after our expected return date, that we must be legally allowed to enter all countries on our itinerary, and that our booking information must match our passport information exactly.
5. Are aware that required travel documents may change at any time without notice and it is our responsibility to be aware of and comply with any new requirements.  This information will be available on the Canadian and US government websites.
6. That further payments for this booking will be taken automatically unless we notify otherwise in writing, and that CruisePlus and travel supplier penalties may apply.  
7. That Travel Insurance – both out of the country emergency medical insurance, and travel and interruption insurance are recommended, and that any decision not to purchase either or both puts me at risk.

NOTWITHSTANDING THE ABOVE, I and everyone who is a part of the booking, have decided to proceed with the travel arrangements that I am asking the Travel Agency to book for me/us and I/we agree that

1. I/we voluntarily assume all the risks involved with such travel, whether those risks are expected or unexpected;
2. in the event that some or all of the travel arrangements are cancelled, interrupted or otherwise affected for any reason, our remedies will be limited to those remedies provided by the travel company physically providing the service.
3. in no circumstances will the Travel Agency, its employees, directors, contractors, and agents, be liable to me/us, or any other person.
